Principal Indications and Effects of Occipital Pull Headgear (I.H.G) in the Treatment of Class II, Division 1 Malocclusion Patients : A Case Report

Extraoral anchorage still is one of the most used and recommended procedures in the treatment of Class II, division 1 malocclusion, which is characterized by maxillar and/or dentoalveolar protrusion. However, only the appropriated use of extraoral orthopedic forces can determine satisfactory clinical results. The selection of the pull direction (high, medium or low pull) is directly related to craniofacial growth pattern and muscle pattern. Other factors to be considered are the growth stage and the face and teeth development, the forces intensity to be applied and the patient cooperation. All types of extraoral pull are beneficial to the patient, since correctly indicated and used, avoiding in many instances dental extraction. The purpose of this study is to address the main indications and effects of the occipital pull headgear (I.H.G.) as well as to present a clinical case treated with this procedure associated to the nonextraction fixed orthodontic therapy.
