Many orthodontic appliances can be used to treat a Class II, Division 1 malocclusion. The correct anchorage indication is directed related to the diagnosis interpretation and treatment planning. Maxillary molar distal movement in vertical pattern patients could be needed, although it doesn’t increase the treatment prognosis. It should be done carefully, not increasing the vertical facial pattern. The paper does a pertinent literature review and presents two clinical cases, both high-angle Class II, Division 1 malocclusion. They were treated with fixed appliance therapy, four first bicuspids extraction and two different kind of extraoral anchorage: cervical and occipital.

Keywords: Class II treatment. Extraoral anchorage. Fixed appliance.
