A Simplified Approach for Class II, Division 1 open Bite Malocclusion Treatment: A Case Report

Currently orthodontics presents a great variety of appliances in the preventive and interceptive areas, as well as in the corrective one, providing the orthodontists with many options, although raising many doubts as to planning mainly when it comes to extrabuccal anchorage. Due to these facts, the orthodontist must, prior to elaborating a planning, evaluate all clinical and cephalometric characteristics aiming at selecting the indicated appliances, by observing its advantages and disadvantages, as well as the need to perform certain modifications in the appliances for its adaptation in specific cases, thus helping during mechanics. Even though the extraoral appliance was developed long ago, and nowadays many orthodontists refuse to use it, due to its disadvantage in relation to patient cooperation, in many cases its use is necessary to provide result following treatment, which is the most important aspect for most parents and patients. Therefore, this article aims at demonstrating the correction of a Class II, Division 1 malocclusion in the mixed dentition with vertical growth pattern, protruded maxilla and markedly retruded mandible, with anterior open bite. A combined extraoral appliance was utilized and for its retention the EBA (IHG) with a modified active lip bumper and after this phase, the fixed orthodontic appliance, giving emphasis to maxillary growth restriction and normal mandibular recovery. It was concluded that this appliance is quite effective, when properly indicated in relation to the patient’s facial characteristics and his/her cooperation level.
