Patient compliance influence in treatment planning and treatment time of Class II malocclusions

Orthodontic treatment alternatives in cases presenting a complete bilateral Class II malocclusion are varied and may include extractions or not. When the option includes extractions, sagital correction of Class II malocclusion is enormously facilitated by the choice of two upper premolar extractions. When the choice excludes extractions and priorize posterior dental distalization, it seems evident a greater mechanical need and mainly a greater demand for patient cooperation so that the expected results can be reached. Therefore it was decided to evaluate and compare the level of cooperation demonstrated by 73 patients showing a complete bilateral Class II, planned without extractions in group 1 or with 2 upper premolar extractions in group 2, and then verify if the initial therapeutical plan associated with the patients cooperation level influenciated final results. The results demonstrated that the initial therapeutical decision based on the sagital correction of Class II malocclusion without extractions, using extraoral distalization forces, was clearly dependent of patient cooperation, forcing this group into a reavaluation in order to reach treatment goals, consequently extending significantly the duration of orthodontic treatment.
