Treatment of osteomyelitis with condyle involvement in children

Introduction: Osteomyelitis is an infectious process that begins at the medullary bone. It is established when pus obstructs blood supply in the medullary cavity. The mandible is the most affected bone in the maxillofacial region. This article shows a survey of 10 cases of osteomyelitis with condyle involvement. Methods: This is a retrospective, case series, study. A total of 10 osteomyelitis cases with involvement of the mandibular condyle were evaluated. Variables such as sex, age, affected tooth, condyle involvement or not, evolution to condyle ankylosis and the need for reconstruction were considered. Results: Patients’ mean age was 8.4 (minimum of 3 years and maximum of 17 years). 60% were males, all of which had permanent or deciduous molar as the affected tooth. here was 100% condyle involvement, and only one case presented evolution to condyle ankylosis. Three cases required reconstruction with costochondral graft. Conclusion: It is suggested that aggressive surgical approach associated with antibiotic therapy and rigorous physiotherapy is the most appropriate method to treat these patients.
