Rehabilitation of atrophic maxilla remains a major challenge for surgeons even today. his challenge becomes even greater when it comes to edentulous patients, who suffer from intense bone resorption processes, which results in Class III or severe deformity. he patient sought rehabilitation with dental implants, but was presented with severe atrophy of the alveolar ridge, associated with large discrepancy between the jaws. Treatment plan consisted in orthognathic surgery (maxillary advancement and mandibular setback) associated with bone block grafts so as to increase thickness of the alveolar ridge. Subsequently, dental implants were installed under proper conditions and proportions. Orthognathic surgery carried out through maxillary advancement and mandibular setback establishes a more harmonious relationship between the jaws, thereby enabling implant placement in proper position and proportions. his allows a prosthesis to be installed over implants without further compensation or extensions in resin, therefore, rendering rehabilitation more predictable, safe and lasting. Orthognathic surgery and jaw reconstructive procedures proved to be of utmost importance for the success of the treatment plan.
