Non-surgical management of recurrent TMJ dislocation

Recurrent dislocation of the temporomandibular joint (TMJ) is a chronic disease in which the condyles move forward beyond the articular eminence, resulting in pain and inability to close the mouth. here are conservative and surgical treatment options. he aim of this article is to report a clinical case of dislocation of TMJ treated with arthrocentesis associated with injection of autologous blood. A female, 42-year-old patient with history of TMJ dislocation for the past two years complained that the symptoms worsened in last three months, with 3-4 episodes of TMJ dislocation per week. Physical examination revealed that the patient presented preserved mandibular movements and pain on palpation of the muscles of mastication. Maximum mouth opening was of 34 mm. Radiographs revealed no anatomical abnormalities. Treatment planning included arthrocentesis and injection of autologous blood, 4 ml injected in the upper joint space and 1 ml in the peri-articular region bilaterally, under local anesthesia and sedation. he patient had an uneventful recovery without new episodes of TMJ dislocation after one year of follow up. Compared to eminectomy, this treatment could be performed in an outpatient setting and under local anesthesia, which reduces the costs. therefore, it could be an alternative treatment for this disease. his treatment allowed recovery of physiological functions of mouth without recurrence of dislocation.
