Management of TMJ dislocation with blood injection

The injection of autologous blood in the temporomandibular joint (TMJ) was recently reintroduced as a treatment modality for chronic recurrent TMJ dislocation. The performance of this procedure by means of the arthroscopy technique is not usually described in the literature. A 53-year-old patient presented with bilateral chronic TMJ dislocation. Treatment plan included injection of autologous blood with the aid of an arthroscope. This procedure was performed on both TMJ joints. Postoperative follow-up of 2 years revealed that the patient had masticatory function restored and was free from dislocation. The concept of autologous blood injection follows the pathophysiology of hemarthrosis because trauma in the condylar region or TMJ surgery can lead to fibrous or bony ankylosis. This injection can lead to the formation of fibrous tissue in the TMJ region. Since there is no tissue dissection, postoperative complications are minor or inexistent in this method. In this clinical case, chronic TMJ dislocation was successfully treated by autologous blood injection. The use of arthroscopy proved to be an effective alternative treatment.
