Treatment of facial fractures immediately after extensive laceration

Mandibular fractures are among the lesions most commonly found in trauma treatment centers. The main causes are traffic accidents and physical aggression, with young male adults being the most affected. A clinical case will be presented describing immediate care and surgical treatment to which the patient was subjected. The patient had suffered physical aggression to the face with a pointed instrument. Male, 34-yearold patient arrived at Netto Campello Hospital in Sertãozinho, São Paulo, Brazil, with facial bleeding. Clinical and computed tomography scanning evaluation revealed fracture of the right mandibular ramus, with traumatic odontectomy of maxillary teeth on the same side and dentoalveolar fracture on the left side, in addition to extensive laceration of the face. The patient was subjected to general anesthesia with nasotracheal intubation. After hemostasis, reduction and fixation were carried out with plates and screws, and suture of the whole area was performed. Three years after surgery, the patient presented adequate soft and hard tissues repair, adequate mandibular movement, mouth opening of 40 mm, restored occlusion and normal drainage of saliva through a parotid duct. Discrete motor deficiency of the facial nerve was still present at the upper lip.
