Objective: To present a modified surgical guide for maxillomandibular repositioning with convenient adjustment, low cost, easy reproduction and manufacturing for use in orthognathic surgery; in addition to conducting a literature review regarding modifications of orthognathic surgical guides. Methods: Technical description of the modified surgical guide manufacturing. Results: It was demonstrated an adaptation for manufacturing a surgical splint to orthognathic surgery. Conclusion: The modification described in this paper may be adequate to stabilize the surgical guide during the trans-operative period, reducing the time required for maxillomandibular fixation and keeping the device postoperatively in position when performing segmented surgery. In addition, it reduces the risks of occupational accidents associated with the use of steel wires for the maintenance of the guides.
