Mandibular defects can happen for a variety of factors, including benign or malignant tumors. Failure to repair these mandibular defects can cause significant psycho-logical impairment, reduced masticatory capacity, difficulty in speaking, and affect the patients quality of life. This paper will discuss the fibula free flap, the most widely used and considered the gold standard choice in mandibular reconstructions. Thus, this study reports a case of mandibular reconstruction using a fibula free flap in a patient who underwent extensive bone resection with disarticulation due to multicystic ameloblastoma. At 14 months postoperatively, the receiving area looks excellent, the patient has a perfect mandibular contour, adequate mouth opening, without deviations from the midline, without painful complaints. There were no complications in the fibular donor area. The use of free bone flaps in extensive mandibular reconstruc- tions is an excellent option, requiring an experienced interdisciplinary team with adequate planning to achieve a satisfactory aesthetic and functional results.
