All-ceramic restorations for complex cases

Aesthetic solutions are becoming increasingly challenging, considering both the level of demand by patients and the latest available materials and techniques, which allow to achieve highly satisfactory results. Added to this scenario and seeking for prosthesis with more stable occlusal patterns, the patient also wants maximum restoration of aesthetics, often already compromised by reduction in vertical dimension of occlusion. The objective of the present paper was to present an alternative for prosthetic rehabilitation planning and describe the aesthetic rehabilitation in complex cases of patients with parafunction and alteration of the vertical dimension of occlusion. Metal free restorations for complex cases became a totally viable alternative as a treatment option for anterior and posterior segments, due to the introduction of advanced technology, high-strength ceramic materials and adhesive cementation technique. After finishing aesthetic and functional rehabilitation of a patient with parafunction it is necessary to use an occlusal splint to act on TMJs inducing the condyle to a correct position in the condylar fossa, and to prevent the occurrence of unwanted fractures and cracks in all-ceramic restorations. A case report with 5-years follow-up will be presented.
