Cytotoxic effect of bleaching agents on dental pulp: an in vivo study

Objective: This study aimed to evaluate the histological changes of the pulp tissue after using different bleaching techniques. Methods: In a split mouth study, premolars of 40 patients were bleached with 10% and 16% carbamide peroxide and 38% hydrogen peroxide with and without light application. Patients were randomized into four groups: Group 1 (G1) = premolars bleached with 10% carbamide peroxide for 14 days; Group 2 (G2) = premolars bleached with 16% carbamide peroxide for 14 days; Group 3 (G3) = premolars bleached with 38% hydrogen peroxide in three applications of 15 minutes each; and Group 4 (G4) = premolars bleached with 38% hydrogen peroxide in three applications of 15 minutes each associated with three minutes of light activation. The control group comprised contralateral teeth. After treatment, the teeth were extracted, cleaned, had their roots sectioned in their most apical portion, and were stored in 10% neutral buffered formalin. The teeth were prepared for histological analysis and data were subject to statistical analysis. Results: G1 showed no significant changes in dental pulp; G2 showed mild inflammation; while in G3 and G4 moderate to severe inflammation was observed. Conclusion: Tooth bleaching techniques employing high concentrations of hydrogen peroxide are more aggressive to the dental pulp.
