Biomechanics for distalization with extra-alveolar mini-screws in Class I patient with bimaxillary protrusion

Introduction: Bimaxillary protrusion patients present anterior projection of the anterior teeth in the skeletal base, with exaggerate anterior inclination, protrusive lips and a convex facial profile. The use of mini-implants (MIs) to address anchorage needs have been widely utilized for anchorage reinforcement purpose placed within the dentoalveolar region, especially between the roots for the treatment with premolars extractions in bimaxillary Angle Class I protrusion patients. However, insertion sites other than the root areas allow for more versatility of orthodontic tooth movements since the roots do not interfere with tooth displacement. Objective: The objective of the present study was to present a clinical case of a Class I malocclusion patient with bimaxillary protrusion in which mini-screws were inserted in the infrazygomatic crest region and in the buccal shelf. Conclusion: The distalization of maxillary and mandibular dentitions using an extra-alveolar skeletal anchorage seems to be a good approach for the treatment of moderate Angle Class I bimaxillary patients.
