Dentoalveolar distalization biomechanics using mini-implants in the palate and an individualized device

Frequently indicated for space creation and correction of the molar relationship, distalization of maxillary molars has been the subject of clinical and scientific studies. In the search for anchorage control and optimal biomechanics, therapeutic options associating bone anchoring and biomechanical devices arise. The present article, based on the pertinent literature, weaves biomechanical and clinical considerations and presents the result of distalization of upper molars using a laboratory device made with rigid structure by means of laser welding and supported on mini-implants installed on the palate. The dental movement in the distalization phase was three-dimensionally evaluated by means of comparing the tomographic cephalometry and superimposition of images obtained with the pre- and post-distalization intraoral scans. The results demonstrated a 4.15 mm distalization in the right upper second molar and 5.64 mm in the left. The maxillary first molars and premolars also suffered distal movement, and the maxillary incisors showed a trend of lingualization. The use of mini-implants installed on the palate and a guided biomechanics with the use of laboratory devices is a viable option for distalization of upper molars.
