Introduction: Mandibular deficiency is present in more than 70% of cases of Class II maloc- clusion and in some cases it is associated to transverse maxillary deficiency that demands both appliances for mandibular advancement and maxillary expansion. Description: This case report aims to describe the treatment of a male patient aged 11 years 7 months, with the dental characteristics aforementioned and pos-pubertal spurt stage, who was treated with an inclined bite plane of acrylic resin covered with a metal track, promoting mandibular advancement, associated with a Hyrax expander supported by mini-implants along the palatal raphe, expanding the suture in a parallel manner. In a few months, the Class II relation was corrected by dental alterations of incisors inclination, leveling the curve of Spee and increasing the upper intermolar distance. Conclusion: The hybrid appliance was efficient in sagittal, vertical and transverse correction, posing as a new approach for early intervention of Class II malocclusion.
