Introduction: The aim of this study is to present, through the analysis of a case report, a protocol for diagnosis and treatment of patients with dentofacial deformities, candidates for orthodontic- surgical treatment. This treatment requires careful analysis of facial aesthetics, occlusion and patient’s breathing pattern. Accurate diagnosis is critical to the construction of a treatment plan, prepared jointly by surgeons and orthodontists. The 3D imaging exams advances in the last decade have allowed an increase in the accuracy of diagnosis and the implementation of an accurate surgical planning. Currently the virtual planning is an important tool for performing the ortho-surgical treatment. The virtual surgery has greater accuracy, improving the capacity of treatment plan’s reproduction in the operating room. However, it is important that protocols for virtual treatment are established and improved.
