Introduction: Asymmetrical malocclusions always represented a challenge to the clinicians, since they can be caused by a number of skeletal, dental and dentoskeletal factors. It is important to differentiate between dental and skeletal asymmetry before establishing a predictable force system for treatment. The use of mini-implants (MI) to address anchorage needs in the modern orthodontic practice have become an important tool for the orthodontists. They have been widely utilized for anchorage reinforcement purpose placed within the dentoalveolar region, especially between the roots. However, insertion sites other than the root areas allow for more versatility of orthodontic tooth movements since the roots do not interfere with tooth displacement. Objective: The objective of the present study is to present a clinical case of an asymmetrical malocclusion (Class II division 1, subdivision) in which a mini-implant inserted in the infrazygomatic crest (IZC) region was used for the correction of the maxillary asymmetry trough unilateral distalization. Conclusion: The unilateral molar distalization biomechanics using skeletal anchorage can be a predictable mechanics, which depends on minimal patient compliance and presents minor side effects, being an alternative to the use of mini-plates.
