Atypical extractions in adult patients for orthodontic treatment purpose

Other extractions than of premolars to solve malocclusions with crowding are not usual in orthodontic therapy, but can be indicated in some specific cases, specially to treat adult patients that seek for shorter treatment time and practical solutions to lead with limitations related to age, periodontal health, extensive restorations, prostheses and implants, situations that is common to this group of patients. Atypical extraction indication involves traumatic teeth, absence of antagonistic or contralateral tooth, impacted teeth difficult to traction, strategies to simplify mechanics and reduce treatment time aiming at aesthetic objectives and to adjust tooth size discrepancies. With coherent and individualized treatment planning it is possible to achieve good functional and aesthetic results using mechanics focused on localized treatment.
