Orthodontic treatment in adult patients with periodontal disease is part of the daily clinical practice. With the increasing life expectancy and concern regarding aesthetics, in addition to accessibility to orthodontic treatment, the number of adults in search for orthodontic treatment has greatly increased. In their vast majority, these patients require interdisciplinary treatment, in which the maintenance of periodontal health is of paramount importance. When the problem goes beyond teeth position and affects facial aesthetics and breathing pattern, the importance of orthognathic surgery is enhanced. In this context, the aim of this article is to present, by means of a clinical case, the treatment of a adult patient presenting severe Long Face and unfavorable breathing pattern, who had significant periodontal sequelae and was submitted to orthodontic treatment associated to orthognathic surgery.
