Clinical aspects of rapid maxillary expansion anchored in mini-implants: case report

Introduction: This case report describes the protocol of miniscrew-assisted rapid palatal expander (MARPE) used in correction of the maxillary transverse discrepancy. This innovative modality of treatment obtains dentoalveolar and skeletal effects as one non-surgical maxillary expansion option. Case report: 14-year-old female patient, with bilateral posterior crossbite. The MARPE appliance used is a bone-borne maxillary expander, therefore included bands in the first molars besides four mini-implants. The forces generated by the activation of the expander screw induced the opening of the medial palatine suture that presented with partial fusion. Results: Pre- and post-treatment CT scans showed an increase of 2 mm in intercuspid distance and 8 mm in intermolar distance; including a skeletal effect of 1 mm. Vestibular bone cortices were preserved. Conclusions: The incorporation of mini implants anchorage to obtain maxillary expansion aims to reduce the need for surgical interventions, as well as the side effects of excessive buccal tipping of the posterior teeth.
