Late supernumerary teeth: something else to worry about

Introduction: Dental abnormalities are a chal- lenge to be managed by orthodontists in a large number of patients. Supernumerary teeth correspond to 8-9% of anomalies and do not appear to be associated with other changes in shape, structure, size or position. They are of genetic-environmental etiology or due to hyperactivity of the dental lamina, usually with a single manifestation and, when multiple, more commonly related to cleiodocranial dysosotosis, Gardners syndrome or cleft lip and palate. Many supernumerary premolars have a late formation, approximately 7 to 11 years after the formation of the numerical series, which coincides with the time of orthodontic treatment for some patients, with the possibility of recurrent formation, being necessary the radiographic monitoring during and after orthodontics. The presence of super- numeraries can hinder orthodontic mechanics, cause resorption of the roots of neighboring teeth, and cause diastemas, but cystic degeneration is relatively rare. Objective: The purpose of this article is to review the literature on the etiology, diagnosis and treatment of supernumeraries with late formation, illustrating with three clinical cases. Conclusion: There is no consensus on the best time for intervention. The decision between cause resorption of the roots of neighboring teeth, and cause diastemas, but cystic degeneration is relatively rare.
