Is the orthodontic treatment of an impacted permanent canine with alveolodental ankyloses possible?

Abstract: Impaction of maxillary permanent canine shows low prevalence in population and usually occurs unilaterally, palatally and in females. The most common treatment in the permanent dentition is the orthodontic-surgical traction. However, treatment prognosis becomes unpredictable when the impacted canine is associated to an alveolodental ankylosis. The aim of this article is to present a case report of a 25-year-old male patient who was diagnosed with palatal impaction of maxillary permanent canines, in which one of the canines was diagnosed with alveolodental ankylosis during treatment. Attempting to solve the case, a surgical luxation of this tooth, followed by an immediate orthodontic traction with low and continuous forces was performed. Despite the ankylosed canine suffered pulp necrosis and slight periodontal and root alterations, both maxillary permanent canines were orthodontically moved to their respective positions in the dental arch. The procedure has contributed to achieving satisfactory results regarding esthetics and function. In conclusion, the orthodontic-surgical management was effective for the treatment of both the ankylosed and non-ankylosed canines.
